Documentation – General Guidance

Your documentation helps SDS better understand your experiences and identify accommodations that work for you. The guidelines below explain the type of information and credentials we look for so we can make an informed decision and ensure the process is as smooth as possible.

1) Description of Disability

  • Diagnosis and ICD‑10 or DSM‑5 code
  • Functional limitations in an educational setting
  • Suggested reasonable accommodations aligned with documented limitations
  • Current and relevant documentation (typically within 3 years)

2) Clinical Credentials

  • Documentation on provider letterhead
  • Clinician’s name, practice name, and state license/certification number
  • Provider must have training related to the diagnosis

3) Additional Notes

  • If requesting accommodations for multiple disabilities, provide documentation for each.
  • If documentation is incomplete or outdated, SDS may request additional records.
  • Prior records (e.g., IEP, prior school accommodations) may be submitted but may not alone validate current need.
