When applying for the New York State Bar or other states, you may be required to submit Certification Forms before or after passing the bar. Forms must be submitted to the Office of Registration and Student Records for completion.

New York State Bar Admission
When applying for admission to the New York State Bar, several forms must also be submitted with your application.
Your Form Law School Certificate and Skills Forms must be submitted to the Office of Registration and Student Records once you have entered your information. Also, please complete a FERPA Release Form to authorize the Office of Registration and Student Records to release your confidential information to the relevant Department.
Documents Submitted to Registrar’s Office Checklist
- Form Law School Certificate (Forms are processed in the order received)
- Skills Affidavit Form (Graduates prior to 2016 or from 2020 are exempt from this)
- FERPA Release Form
- Please add this to the authorization on FERPA form: Email submission of my Form Law School Certificate, which contains my Social Security Number as well as my address/phone information.

Form Law School Certificate
When submitting your Form Law School Certificate, please remember to sign the Certificate before sending it to the Registrar’s Office.
Skills Affidavit Form
*If you have graduated from the law school prior to 2016 or if you graduated in 2020, you are exempt from having to submit the Skills Affidavit Form.
When filling out the Skills Form, you will select Pathway 2.
If you are a PBS graduate, you will select Pathway 3 on the skills form, have it notarized, and submit it directly to your department.
Pro Bono Requirement
You will need to complete the following form for each pro bono project that you will rely on to satisfy the Pro Bono Requirement. Send the Form Affidavit of Compliance to your clinic supervisor for completion (50 Pro Bono Clinic Hours)
