Find the Forms You Need

The Registrar’s Office provides a range of forms to support your academic progress and administrative requests. To help you find what you need quickly, forms are grouped by purpose—like registration updates, academic program changes, and records requests.

Be sure to read the instructions for each form carefully. If you’re unsure which form to use or have questions, reach out to our team—we’re here to help.

Course Registration & Academic Programs

Advanced Clinic Registration (CED, DEF, ELC, MED)

Used to enroll in upper-level clinical offerings like Community & Economic Development, Defenders, Elder Law, or Mediation clinics.

Step 1: Save the form first and then complete it.
Step 2: Get the professor’s approval.

Advanced Trial Practice

Permission to register for the advanced-level trial practice course.

Step 1: Save the form first and then complete it.
Step 2: Get the professor’s approval.

Audit Form

Request to attend a class without receiving a grade or academic credit, the grade of “AUD,” will be assigned to the course. Permission from the faculty member who is teaching the course is required.

Audit Form 

Independent Study Application & Registration Form

To request Independent Study credit, complete this form and obtain approval from the Academic Dean. Students may earn up to 3 credits toward graduation through faculty-supervised research. Additional credits require exceptional circumstances. Note: internships or clerkships alone do not qualify—projects must involve original research under direct faculty supervision. Catalog criteria apply.

Register for Law Review credit while serving as an editor. For requirements and specifications on law review, check the current semester course booklet.

Register for credit while participation in CUNY Law’s Moot Court program.

Teaching Assistant Form

For requirements and specifications on teaching assistantships, check the current semester course booklet. Teaching Assistantships are available for required courses and the Summer Law Institute only.

Dual Degree Program Form

We offer three dual degree programs: JD/MPA in Law and Public Accountability, with the John Jay College of Criminal Justice; JD/MA in Forensic Psychology, with the John Jay College of Criminal Justice; and JD/MIA with City College’s Colin Powell School for Civic and Global Leadership.

Each Master’s program has slightly different requirements, but each dual degree program allows a student to earn the two degrees in less time than it would take to complete each separately, because the programs have agreed to recognize some of the classes from the other program toward its degree.

Leave of Absence Request

Apply for temporary leave from the Law School, for medical or personal reasons.

Used exclusively by first-year students to request permission to drop a course, usually under narrow circumstances.
